Tennessee Change of Address Forms

The DOS and DOR requires residents to notify a change of address within 10 days of moving to the new address. You could complete this obligation either online, by mail, or in person. While updating your address, you need to provide the following information:

• Full name
• Birth date
• Driver’s license or ID number
• Social Security Number
• Previous address
• New address
• New county
