Note that there are forms that cannot be found online, which means that you may be required to visit your local DMV office for collection.

Use this form to update your address information so it could be reflected on your driver’s license or vehicle records with the OR DMV.

The Oregon DMV requires resident to notify a change of address within 30 days of moving. You could complete this procedure either online, by mail, or in person.

By Mail or In Person

Whether you need to change your address by mail, complete the Form 735-6438 and mail it to the address that shows on the form.

If you prefer to update your address information in person, fill out the form and take it to your local OR DMV branch.
