Below, you’ll find a list of available forms which were classified by categories for your own convenience.
Note that some forms are not available online so you are required to contact the FL DHMSV by phone or visit one of their divisions in person.

|Certification of Driving Experience of a Minor – Form HSMV 71143||Parents and other legal guardians can use this form to notify the DHSMV that a minor under their custody has completed the required amount of practice driving hours to apply for a Florida license.|
|Parental Consent for a Driver Application of a Minor – Form HSMV 71142||Parents and guardians must sign this document when a minor applies for a FL driver’s license.|
|Driver License Records Request – Form HSMV 90511||Request a copy of your driving records for you or any other person who may require it by law.|
|Fraud Investigation Requests – Form HSMV 72068||Victims of identity theft or fraud involving their driver’s license must submit this to the FL DHSMV.|
|Mature Driver Vision Test – Form HSMV 72119||Use this form if you are mature driver who needs to take an eye exam before renewing your driver’s license. Note that this form must only be completed by an eye doctor.|
|Report of Eye Exam – Form HSMV 72010||Submit this form if you failed your eye exam while renewing your driver’s license in FL. Note that this form must be completed by an eye doctor.|
Florida Change of Address Forms
Whether you need to changer your address, the Florida Department of Highway Safety and Motor Vehicles (FL DHSMV) and the Division of Driver Licenses require drivers to consider the following:
Visit the FL DHSMV GoRenew System and provide:
- FL driver’s license or ID card number
- Date of Birth
- Last 4 digits of your Social Security Number
- Pay a $25 fee
- Go to a Division of Driver License agency and provide:
- Proof of citizenship or legal presence in the US
- Social Security Number
- Proof of residency in Florida
- Pay a $25 fee
