Certificate of Transfer Without Probate – Form MVD-10011 | Download
Use this form to transfer ownership of a car from a deceased person’s car to an heir or spouse without the use of a probate. Note that this form must be notarized prior to submission.
Duplicate Certificate of Title Application – Form MVD-10901 | Download
Submit this form to obtain a replacement NM vehicle title with the MVD.
Non-Use of Vehicle/Out of State Vehicle Insurance Affidavit Form – Form MVD-11268 | Download
This form is used to verify that a vehicle that has been registered in NM is not being operated or is out-of-state.

New Mexico Change of Address Forms
Change of Address Request – Form MVD-10284 | Download
Submit this form to the MVD when you change your address.
Note that you are required to update your address information within 10 days of moving to your new location. The NM MVD allows residents to change their address either by email, by fax, mail, or in person.
