Louisiana Change of Address Forms

Visit your local OMV office to change your address on your driver’s license or vehicle registration. You must do this procedure in person and pay the required fees.

Driver’s license/ID card address change
The LA OMV requires residents who move to a new address to notify them within 10 days. You must visit your local OMV agency in person and provide the following:

• LA Driver’s license or ID card
• Proof of auto insurance
• Pay the required fee ($13 – $17)

Vehicle registration address change
The LA OMV gives you the chance of changing your address on your vehicle registration either in person or by mail. Make sure you provide the following:

• Written request if changing your address by mail
• Your vehicle registration certificate
• Pay the required $12 fee
